The Medical College of Wisconsin OESO-SEMPIRE Pilot Center, anchored in the Pepsin and Reflux Laboratory and led by Nikki Johnston, is internationally recognized for redefining reflux disease beyond acid. Our work established pepsin as a central mediator of epithelial injury, inflammation, and carcinogenesis across the aero-digestive tract, reshaping the conceptual framework of laryngopharyngeal reflux (LPR). Building on this foundation, we are the first group worldwide to advance a targeted LPR therapeutic into investigator-initiated Phase II clinical trials, focusing on pepsin inhibition for PPI-refractory disease. Our multidisciplinary team of laryngologists, ENT scientists, and speech-language pathologists collaborates across gastroenterology, pulmonology, and global partners to translate mechanistic discovery into first-in-class diagnostics and therapies.
